The following are some elements which are normally found in a continual improvement register template. The user may customize the form based on his or her needs.

1. Identification and Source Information

  • Improvement ID / Reference Number: A unique number assigned to identify the improvement.

  • Date Identified: The date when the improvement opportunity was first considered.

  • Source of Improvement: The source of the improvement opportunity e.g. Internal Audit (IA-001), Management Review (MR-003), Customer Feedback (CF-012), Nonconformity (NC-005), etc.

  • Identified By: The name of the person or department which identified the improvement opportunity.

2. Description and Proposed Action

  • Description of the Improvement Opportunity: A brief description of what the improvement opportunity is about.

  • Proposed Action / Solution: The proposed solution for the improvement opportunity.

  • Reason for Improvement (Optional): A brief reason for the improvement e.g. "to reduce error rate,"to enhance customer satisfaction," "to streamline process," etc.

3. Responsibility and Deadlines

  • Responsible Person / Department: The person or department responsible for implementing the proposed solution.

  • Target Date for Completion: The target date for completion of implementation of the proposed action.

4. Progress and Verification

  • Actual Completion Date: The actual date when the proposed solution was implemented.

  • Status: The status of the improvement opportunity.

  • Verification of Effectiveness: How the implemented solution was checked and what the results were.

  • Verification Date: The date of verification.

  • Verified By: The person or department which carried out the verification.

5. Review and Closure

  • Lessons Learned / Notes: Additional notes about the improvement opportunity.

  • Review / Closure Date: The date when the improvement opportunity was reviewed and closed from the register.

  • Management Review Reference (Optional): The reference to the management review meeting.

Implementing and Using Your ISO 9001 Continual Improvement Register

Once you have the register, it is necessary to implement and start using it.

The following are some pointers to help you implement the register:

  1. Integration with QMS Activities: Ensure that the register is integrated with your QMS activities and that you update it as soon as improvement opportunities are identified during internal audits, management reviews, or nonconformity closure activities.

  2. Regular Review and Updating of the Register: The register should be reviewed and updated at regular intervals. Decide on how often you should update and review the register e.g. weekly, monthly or quarterly.

  3. Assign Responsibility for Register Maintenance: Assign responsibility to a particular individual or department to take care of the register.

  4. Train the Staff on the Use of the Register: Train the staffs on the need and the use of the register and encourage them to identify and fill in the improvement opportunities.

  5. Analyze the Data in the Register: Study the data in the register on a regular basis. Find out the areas in the QMS which give rise to many improvement opportunities.

  6. Celebrate the Achievements and Communicate Them to the Staff: Motivate the staff by communicating the achievements made through the implementation of the improvements.
