8D Report Template

by Poorva Dange

Introduction

Problems are inevitable, and an organization’s dedication to quality improvement will help it through any situation. One of the methods to do it successfully is the eight disciplines of problem solving (8D). Using an 8D report template helps companies ensure that they understand what caused the issue, solve it, and prevent it from happening in the future. Moreover, the document can be used for ISO 9001 compliance.

8D Report Template

What is an 8D Report? The Meaning and Purpose

8D is a team-oriented method initially created by Ford Motor Company to systematically eliminate problems by analyzing the root causes and finding the best corrective actions. It was designed to address the most recurrent, difficult-to-solve issues a company faced. Using the 8D approach makes an organization more receptive to the ideas of continuous quality improvement, as the report helps the team not only solve the identified problems but also prevent future ones.

The Benefits of an 8D Report Template for Quality Management

A good 8D report template ensures that the company addresses the problem correctly. Furthermore, it helps the auditor understand what actions were taken to solve it. In addition, the document contains detailed information about the issue and its impact on the business, which will be helpful for both the company and the client. Another advantage is that an 8D report serves as excellent training material showing the staff how to deal with quality-related problems in the future. In this way, the document becomes a good practice for the auditor to assess the organization’s readiness to improve its quality performance.

ISO 9001 and 8D: Enhancing Continuous Improvement through Problem-Solving

Since organizations that use the ISO 9001 standard dedicate themselves to quality management, applying an 8D report aligns perfectly with these objectives.

For instance, clause 10.2 of ISO 9001:2015 refers to nonconformity and corrective action. According to this requirement, entities shall react to nonconformities, take actions to control and correct them, deal with the consequences, evaluate whether there are any actions necessary to prevent recurrence, and implement them. Obviously, using an 8D report to analyze the causes and find corrective actions fits this clause.

Moreover, clause 6.1 requires the organization to take actions to address risks and opportunities. It means that entities must analyze the reasons to prevent similar problems from occurring in the future, which is another advantage of using 8D.

Another relevant clause is 10.3, which states that the organization shall continually improve its quality management system. Using the 8D method fits right into this frame, and the auditor will see that the entity truly cares about quality improvement.

Finally, a correctly filled 8D report will serve as strong evidence of the organization’s ability to solve issues that arise in its operations.

The 8D (Eight Disciplines) Report – Everything You Need to Know

8D methodology involves eight disciplines that are described below. An 8D report template usually has sections for each of them.

D1: Team

The first step is to assemble the team. It should have the required specialists who have the necessary expertise in the product, process, or service. The people should identify their roles and responsibilities.

D2: Problem Description

The next step is to describe the problem. At this stage, it is essential to collect all the relevant data. When filling out the 8D report template, make sure to use the 5W2H principle (who, what, when, where, why, how, how much).

D3: Interim Containment Actions

After describing the problem, the team should develop interim containment actions. They will ensure the client gets the needed product or service. After the actions are determined, it is required to verify their effectiveness before moving to the next step.

D4: Determine Root Causes

Then, the people should find the root causes of the issue. At this stage, it is recommended to use various tools, such as Ishikawa (fishbone) diagram, 5 Whys, and failure mode and effects analysis (FMEA). Remember that it is essential to identify not the symptoms but the underlying problems.

D5: Permanent Corrective Actions

When the team has determined the possible root causes, it should choose the most probable ones and develop corrective actions. After that, the people should verify the actions’ effectiveness.

D6: Implement Permanent Corrective Actions

When the corrective actions are verified, it is time to implement them. In this section, it is required to describe the detailed plan of action. Furthermore, the people should clarify roles and responsibility distribution and verify corrective actions’ effectiveness.

D7: Preventive Actions

Next, the team should ensure that the system will not fail again in the future. To do so, the people should make appropriate changes to the processes. At this stage, it is also needed to share the lessons learned with the organization. This way, the company will be able to prevent similar quality issues.

D8: Congratulate the Team

Finally, it is time to appreciate the team for its hard work with bonuses, parties, or other activities.

Essential Elements of a Good 8D Report Template

Depending on the report, a good 8D template will usually have sections with the following elements.

1. Report Header

The header usually contains general information, including report ID, date, problem owner, customer name, and affected product or part.

2. D1: Team

This section contains the names, departments, and roles of the team members.

3. D2: Problem Description

The problem description section refers to the general information about the problem. When filling out this section of an 8D report template, use the 5W2H principle.

4. D3: Interim Containment Actions

This part involves the description of the actions taken to solve the problem.

5. D4: Determine Root Causes

Here, the team should analyze the root causes of the issue. Make sure to use fishbone diagrams, 5 Whys, and other tools.

6. D5: Permanent Corrective Actions

At this stage, the team should describe the actions that will permanently solve the issue. When selecting the most effective course of action, make sure to consider the alternatives.

7. D6: Implementing Corrective Actions

After they are determined, the people should implement the corrective actions. This part of the report should also have a detailed plan of action, responsibilities, and dates. Finally, the corrective actions’ effectiveness should be verified.

8. D7: Preventive Actions

The section should describe the actions that will ensure that the issue will not occur again in the future.

9. D8: Congratulate the Team

The final part of the report should congratulate the team on its hard work.

10. Additional Information

This part of the report usually contains all the additional information, including pictures, that may be needed. Make sure to attach all the supporting documents.

How to Take Advantage of an 8D Report Template and Process

To get the most out of an 8D report template, an organization should ensure that it fully exploits all the benefits of the eight disciplines. To do so, the managers should first train the employees and ensure that they have a good understanding of the concept of continuous improvement. It is also essential to encourage the staff to report any problems they encounter. In addition, the leaders should support the quality managers in their work and take timely and relevant actions.

Another important point is to base all the decisions on facts and data. Although the 8D method encourages involvement in decision-making, some issues still require specialists’ expertise. For instance, when determining the real root causes of the problem (D4), it is essential to rely on the engineers. Besides, timely reaction is also of great importance. Finally, always focus on the quality improvement objectives. To achieve them, make sure that you take all corrective and preventive actions.

ISO 9001

In Summary

Applying an 8D report helps the organization comply with the ISO 9001 requirements, as using this document shows that the entity is willing to continuously improve its quality performance. An 8D report template makes it easier to address difficult problems an organization faces. In turn, this helps the company be more proactive rather than reactive to the issues that arise and better plan its corrective and preventive actions. An organization that properly uses an 8D report template will be able to improve its processes and, thus, product or service quality, which will lead to customer satisfaction and loyalty in the long term.
